How Do I Start?

You want the best for your child. Get started in 5 simple steps.

  1. Pre-register your child. There are two options to "Pre-register."
    • Call 785.232.5005 and ask to speak to Admissions. An Admissions Coordinator will tell you everything you need to do and will answer all your questions.
    • Online - Click here. Be sure to click "Submit" to send your form to us.
      FSGC Admissions will contact you to set up your appointment. Family Service & Guidance Center has an Admissions Clinic that allows your child to be seen the same day you make your first phone call or walk through the doors. More information below.
  2. Complete your Paperwork.
    Click "Admissions Paperwork" and "Print." Fill them out and bring them to your child's first appointment. Doing this before you come to FSGC will save time. If you need help filling out the forms or have questions, call Admissions. If you have problems downloading or printing the paperwork, call Admissions and ask for the forms to be mailed to you.
  3. Provide your child's mental health treatment records.
    If your child has received mental health care services before or if they are currently involved in treatment, contact their providers and ask them to release any records to Family Service & Guidance Center, Attention: Admissions. You may also bring them with you to the first appointment if you like.
  4. Bring your child's insurance card.
    Be sure to bring your child's insurance card with you to the appointment. If you do not have the card, try to get a copy of the card, front and back.
  5. Bring proof of your income.
    FSGC provides a sliding fee scale based on household income if you live in Shawnee County. If you wish to pay on a sliding scale, be sure to bring income verification. Examples of income include:
    Wages (copy of recent check stub or most recent tax return)
    Social Security Benefits
    Public Assistance (Medicaid)
    Retirement/pension (SSI/SSDI)
    Child Support/Alimony - received or provided
    Unemployment Insurance
    Other Income not listed here.

Parents/legal guardians should bring photo identification, such as a driver's license.

  • Please arrive at least 15 minutes before your scheduled appointment.

Answers to frequently asked questions

  • Family Service & Guidance Center is located at 327 S.W. Frazier Ave., near 6th and Gage. If you need help finding us, call 785.232.5005 for directions or Click Here for Map Directions.
  • For the first appointment, the child and at least one parent or legal guardian must attend.
  • Please check in at the front desk when you arrive. The receptionist will review your paperwork and make a copy of your insurance or medical card and identification.
  • At your first appointment, you will...
    • Meet with an Admissions Coordinator to finalize paperwork and answer your questions.
    • You will meet with a therapist at the initial appointment and discuss your child's treatment options.
    • Have a chance to schedule a second appointment, if one is needed.

CMHCs

Contracting Community Mental Health Centers (CMHCs)

If you have a contract with FSGC to use our Crisis Resolution Program or Overnight Respite Care, we encourage you to review the procedure below for accessing CRP or Overnight Respite Care for your client. Links to the needed paperwork are also included.

To refer a client for CRP or Overnight Respite Care, the referring CMHC needs to forward the following completed documents to FSGC:

  • Admission Referral to CRP / Overnight Respite – CMHC Contracts: This document will explain how the program works and how your client can access it. It must be signed by the referring Qualified Mental Healthcare Professional (QMHP), the client and the client’s parent or legal guardian.
  • Screening Document: This form must be completed if the client is being diverted from the State Hospital.
  • Treatment Plan/Plan of Care: FSGC needs the client’s treatment goals and objectives along with their billing diagnosis for this service.
  • Use and Disclosure Form: This form allows CMHCs to exchange information.
  • Contact Information: Please include the CMHC Contact Person’s phone number.

 

FSGC staff members will review the referral and clinical documentation as soon as possible to determine approval for admission. Once a determination is made, FSHC will contact the CMHC. If the admission is approved, the following documents can be completed by the parent or legal guardian. The completed forms can be sent to FSGC in advance or the parent/legal guardian can bring the documents with them at admission.
